Periacetabular Pelvic Osteotomy in Treatment of Patients with Developmental Dysplasia of the Hip

Background. Pelvic osteotomies are widely used for treatment of young active patients with developmental dysplasia the hip (DDH) Type I according to Crowe or type A Hartofilakidis classifications and absence severe degenerative cartilage lesions. Nowadays, Ganz periacetabular osteotomy (PAO) is most common choice surgeons around world in such patients. The aim study was evaluate radiological functional results DDH. Material Methods. single-center retrospective analysis 49 PAO performed 43 aged 36±9 years. evaluation criteria were Wiberg (AW), Lequesne (AL), Tonnis (AT) angles, joint medialization. evaluated using Harris scale, iHOT-12, VAS before 1 year after, presence complications also monitored. Results. average follow-up period 35±15 months (from 1.0 6.9 years). Radiological parameters improved after surgery compared preoperative ones: AW +19.9° (17.1° vs. 37.0°), AT -11.5° (19.9° 8.4°), AL +14.1° (25.1° 39.2°), medialization -5.5 mm (14.3 8.8 mm) (p<0.001). Functional quality life improved: scale +35.6 points (47 vs 83 points), iHOT-12 +40.9 (44 85 pain level -2.8 (5 2 points) Various developed 20 out cases (40.8%). Neurological resolved conservatively (22.4%). direct correlation between surgical DDH childhood development neurological (R = 0.76; p<0.001). In 9 (18.4%), revision required: 3 — total replacement, reorientation acetabulum, 4 arthroscopic fixation anterior articular lip. 93.9% native surfaces preserved. Conclusion. has good reconstructive capabilities sufficient efficiency. operation allows restore coverage femoral head delays replacement provides results.
